The X STOP is the fi rst Interspinous Process
Decompression (IPD) device that is proven to be
superior to non-operative therapy in patients with
neurogenic intermittent claudication secondary
to lumbar spinal stenosis (LSS) in a multicenter
randomized study at 1 and 2 years. The clinical
success rate in the X STOP IPD group was comparable
to that reported for laminectomy. The goal of
the current study was to compare the clinical
effectiveness and direct hospital costs of IPD with the
X STOP implant to those of laminectomy in patients
with LSS.

With the recent FDA Approval of the CHARITE Artificial Disc, and
ongoing studies with a number of other artificial disc designs nearing
completion, artificial disc replacement is fast becoming the leading
edge of spinal surgery, an area that spine surgeons and patients
alike are eagerly embracing. The anterior approach to the spine has
been used for many years for fusions and subsequent redo operations,
`and the complications of these operations are well known to spine
and access surgeons. Artificial Disc replacement is a new technique
that is just beginning to be performed more widely, and the number
of revision or redo surgeries for these discs is small at this time.
There is little doubt however that these numbers will soon be increasing,
and judging from early evaluation of these cases, it may be that
re-operating on total discs may present its own unique challenges
separate from routine fusion cases.

Neurogenic intermittent claudication (NIC) secondary to lumbar spinal stenosis (LSS) is a posture-dependent complaint and it typically affects patients at the age of 50 years or older. NIC is defined as pain or numbness in the buttocks, thighs, and/or lower legs caused by decrease of the spinal canal area and brought on by either prolonged standing or exercise in the erect posture. The symptom is typically relieved by various manoeuvres that flex the lumbar spine, which increases the spinal canal area significantly

The discogram is still a poorly understood procedure but the only test that may tell the Specialist where the pain in the spine is coming from. Sometimes apparently minor abnormalities may cause alot of disability such as these endplate fractures which are not commonly considered a source of pain.
